Day Care
Our Day Care team supports patients who may become homebound or socially isolated due to declining functional status, providing them connection, comfort, and purpose through therapy and psychosocial activities. These activities can be led by volunteers, which enhance patients’ quality of life, provide caregiver respite, and promote social participation. The nursing team also monitors and manages clients’ symptoms regularly.
Eligibility
We support:
- Individuals diagnosed with advanced terminal-illness (cancer or non-cancer), with a prognosis of less than 1 year
- Individuals whose symptoms are stable and controlled, and are functionally able to tolerate sitting and traveling to Day Care
- Clients with physical and psychosocial care needs that require day time support, monitoring and maintenance, including those isolated at home due to advanced illnesses who wish for community and social interaction, and to participate in meaningful activities
- Individuals with chronic advanced lung diseases enrolled in R.I.S.E. (Restitutive. Integrative. Supportive. Empowering), our curated 12-week programme that empowers participants with skills and confidence to self-manage breathlessness on their own in the community through symptom monitoring, physical rehabilitation, self-awareness and confidence building, education and psychosocial support for behavioural change
Referrals and Admissions
Referrals can be initiated by the care team at Tan Tock Seng Hospital. The referring doctor has to determine if the patient meets the admission criteria and resides within the designated areas in central Singapore.
Referrals to be made through the Agency for Integrated Care (AIC)/ BRIGHT portal or by completing a referral form obtained from the Singapore Hospice Council website.
Upon acceptance of referral, a call will be made to the referred patient and family, for further clarification on the service provided and the necessary arrangements to be made for the first visit to Day Care.

- A volunteer-led initiative serving local Kopitiam toast and drinks to bring comfort, friendship and a sense of normalcy to our patients.
- Namaste Care:
- A gentle and relaxing hand and face massage to calm patients’ mind and body (stroking touch massage, calm music, soft lighting and aromatherapy).

Rehabilitation Activities
Alongside daily activities, our rehabilitation therapies bring comfort and calm to patients through gentle care.

R.I.S.E (Restitutive. Integrative. Supportive. Empowering) Programme
In collaboration with Tan Tock Seng Hospital and Ren Ci Hospital, the R.I.S.E Programme is a 12-week programme that equips individuals with chronic lung diseases with the skills and abilities to self-manage their breathlessness at home. Through a multidisciplinary team comprising a doctor, nurses, medical social worker, occupational therapist and physiotherapist at Dover Park Day Care, the programme provides nursing, psychosocial and rehabilitation care to optimise physical functions through functional pulmonary rehabilitation therapy. Thus, enhancing their quality of life and allowing reintegration to communities.
Virtual Reality
With growing research and studies, the use of Virtual Reality (VR) has proven to benefit palliative care patients in the area of pain management, physical therapy and cognitive stimulation. VR provides an immersive and realistic experience where our clients can be transported to another country or place without the need to travel. It also helps motivates patients to increase their physical activities through different virtual games or activities.
Charges and Subsidies
Day Care Service is fully funded at no charge (including transport) , excluding ad-hoc consumables which are MediSave claimable.
Operating Hours
Monday to Friday: 10.00am to 3.00pm
2-way wheelchair transport is provided at pick-up/drop-off point
Breakfast, Lunch and Tea Break are provided.
