Legacy Planning

Advance Care Planning (ACP) is a process where individuals express their values, beliefs, and healthcare preferences for future medical care, especially in situations where they may be unable to make decisions for themselves. It involves documenting these preferences and sharing them with loved ones and healthcare providers.

A Preferred Plan of Care (PPC) is a specific type of ACP that focuses on individuals with terminal illnesses, outlining their wishes for end-of-life care, including life-sustaining treatments and preferences for place of care and death.
